Chemical Request

This is a login for a form where you can request to bring non-supplied chemicals into MRL Cleanroom and/or RaithPrep room.
To use this form, you must have already received training for the lab you wish to use.
You will be asked the following questions for each chemical you would like to use:

• Name of the chemical
• CAS No. for the chemical
• UI No. for Chemical Waste pickup
• Qty of the chemical/Number of containers
• Link to MSDS
• Procedure describing the use of the chemical in the prep room
